Online voter registration forms are available in both English and Spanish and may be downloaded below. To print the online registration form Adobe Acrobat Reader™ is required. Adobe Acrobat Reader can be freely downloaded at www.adobe.com or by clicking here. To request a Voter's Registration form over the telephone please call (585) 753-1550 and a registration form will conveniently be sent to you in the mail to complete and return. To download the English voter registration form please click here. To download the Spanish voter registration form please click here. 1. Complete the entire application forms. 2. Remember to check the box next to the political party in which you wish to enroll. If you do not wish to be affiliated with any party, check the appropriate box. 3. Sign and date your application in blue or black ink (This is Required). 4. Mail (do not fax) the completed application. Send to: Monroe County Board of Elections 39 W. Main St. Rochester, New York 14614 Note: This address is for Monroe County residents only. If you want to vote in an election, you must mail or deliver your completed registration form to the Board of Elections no later than 25 days before the election in which you want to vote. Your eligibility to vote will be based on the date you file this form, and the county board will notify you of such. |
